Abstract

Primary anorectal melanoma is a highly malignant tumor, with a high metastatic capacity and a poor prognosis. We present the case of a 75-year-old patient who was diagnosed with anorectal melanoma, without involvement of the sphincter apparatus, without distant foci. Local surgical resection was performed for symptomatic relief and palliative radiotherapy. Scientific evidence at present does not allow clear guidelines to be established regarding diagnosis and treatment, which is highly controversial. Its treatment is fundamentally surgical, with debate between abdominoperineal amputation and local excision. Currently, there are no clinical trials to evaluate the differences between both techniques. However, many opt for local excision, since the oncological results are similar and the morbidity and mortality rate is clearly better.
